We understand that recovering from surgery can be a challenging and transitional time. Our team of highly experienced healthcare professionals is here to provide comprehensive support. From Skilled Nursing to Physical Therapy, Occupational Therapy to Speech Therapy, Social Work to Pharmacy, and Home Health Aide services, we offer a wide range of specialized care options tailored to meet the unique needs of each patient.
Our team of specialists are dedicated to helping you improve your level of functioning and reach your recovery goals. We understand that each patient is unique, and our therapists will work with you to reach your optimal level of independence and functional ability.
Registered Nurses are skilled at providing post-operative care, safety evaluations, wound care, IV therapy, catheter care, and medication management. They act as a liaison between doctors, pharmacists, patients, and their caregivers to ensure that patients receive the necessary care in their home environment.
Physical Therapists evaluate and treat a variety of conditions, such as pain, injury, neurological impairments, or muscle and bone disorders that limit safe mobility. They develop an individualized care plan that incorporates exercises to build strength, coordination, balance, and endurance.
Occupational Therapists help patients regain their independence in daily activities, such as bathing, grooming, dressing, housekeeping, and meal preparation. They achieve this by providing treatment to improve functional mobility, including the use of adaptive equipment and home modifications.
Speech Therapists work with patients who experience communication and/or swallowing disorders that accompany diseases such as stroke, MS, and traumatic brain injuries.
Social Workers offer assistance to families and caregivers by enhancing coping skills while promoting the social and emotional health of patients. Social Workers have access to a network of community resources to assist in maintaining the patient’s health and safety.
Pharmacists collaborate with Nurses and Therapists to ensure the accurate delivery and management of medication for patients and their caregivers, which is crucial in preventing hospitalization.
Home Health Aides are critical members of our team who provide personal care assistance, such as bathing, dressing, transferring, toileting, and skin care. They work under the supervision of nurses and/or therapists to provide support and care for patients.
